
Director of Care Services
MISSION: To help older adults age in place, by connecting family caregivers to resources and respite that will empower them to be the best caregivers they can be, making Eagle County a great place to LIVE and AGE.
In Eagle County, older adults 65+ are the fastest growing population, which means an increase in the number of family caregivers in our community. Caregiver Connections supports this population, regardless of socio-economic status, through low- or no-cost services, programs and resources.
POSITION SUMMARY: Lead Caregiver Connections staff professional, reporting to Executive Director, that helps older adults and their family caregivers age in place in Eagle County through oversight and support of all services, including respite programs, care consultations, medical supply closet, and other new / emerging programs & resources. This position acts as the lead client care and case management role, fostering healthy relationships between caregivers, care recipients, agency & provider partners, volunteers and other community stakeholders. The Director of Services plays a key role in making Eagle County a great place to live AND age.
KEY RESPONSIBILITIES:
- Lead expansion of Respite Program for new days & location, building off proven model of existing Senior Spot program, as well as management of Care Companion/Activity Buddy volunteer program.
- Oversees client case management, including conducting care consultations (see description of our Care Consultations at https://www.getcaregiverconnections.org/care-consultation) , taking on referrals from local providers, managing follow-up, and ensuring good record tracking.
- Manage Medical Supply Closet (MSC) loan inquiries, donations and equipment tracking. Duties include scheduling appointments with donors/borrowers, filing MSC forms in Charity Tracker, and communicating with community partners for oversized DME needs/donations.
- Manages Charity Tracker database. Ensures Charity Tracker, case management platform, is up to date and produces accurate reports when needed, including routine reporting for use by the Executive Director, Board or other staff as needed.
- Conducts regular (bi-weekly or monthly) case reviews of participants in the Senior Spot program with Respite Program Director, including Exec. Director as needed.
- Develops and leads new outreach initiatives to the Hispanic / Latino community for access and involvement in programs & services (incl. care consultations, respite programs, support groups, medical supply closet, etc.)
- Coordination and liaison with local area providers, care coordinators, in-home care agencies, and case managers, to take referrals, provide reports/feedback to the referral source, and work with partners (like Eagle County Healthy Aging), EC Paramedic Services, etc) for program ideas and implementation.
- Maintains & updates educational information/resources for clients (eg. resource pages, guides, reference materials). Creates new educational materials as needed / requested.
- Supports Parkinson’s Support Group and Dementia Support groups, as needed by current facilitators. Collaborates closely with Dementia Support Group leader / facilitator for the Dementia Support Groups, including gathering attendance and contact info / inputs for both support groups for entry into Charity Tracker. Maintains email list and contact with Parkinson’s of the Rockies. Provides education & support to those diagnosed and their care partners, maintaining relationship with sources of referrals and maintaining knowledge / staying current with diagnosis info, treatment, etc..
- Manages existing programs, resources and processes, evaluating effectiveness and identifying new resources, tools and/or services that might be needed. Works with outside partners as needed.
- Supports development of new program materials, processes and procedures as needed, working closely with Exec Director, Respite Program Director and other relevant partners. Evaluates program materials and updates periodically as needed.
- Makes recommendations for individuals suitable for applying to Caregiver Assistance Fund, with recommendation to Exec Director and to the Board. Handles communication and reimbursement process.
-Coordinates and manages Eagle County Caregiver Classifieds (private Facebook group platform to connect paid skilled caregivers with care recipients in need of in-home care). Recruitment, assistance to users, tracking and monitoring.
- Assists with other Caregiver Connections programs, services and resources as necessary, to support the organization’s team (eg. Senior Spot, Aging Well Speaker Series, Medical Supply Closet)
- Assists in development of Annual Report, Impact Report and other marketing & development / fundraising materials, working closely with Exec. Director.
NECESSARY SKILLS & BACKGROUND EXPERTISE:
- Background in human/health/social services; ideally with bachelor’s degree ideally in related field (desired but not required).
- Experience working with older adults, including experience with assessments (physical, socio-emotional, medical);
- Spanish-language skills are highly desirable, but not required.
- Understands the issues and concerns of older adults, particularly those relevant to family caregivers. Motivated/interested in assisting older adults to maintain their independence and improve their quality of life – both caregivers and care recipients.
- Knowledgeable about dementia, cognitive impairment and other age-related diseases or willingness to learn (i.e. participate in training programs).
- Excellent interpersonal skills. Ability to communicate clearly and effectively to impart educational information. Strong reflective listening & empathetic communication skills. Very good problem-solving and decision making skills.
- Good computer skills, especially for database/client records. Understands importance of record keeping and of complying with privacy requirements (eg HIPAA).
- Motivated to learn. Desire to keep abreast with latest information & local resources and willing to watch educational videos and read materials to better understand the various forms of dementia and other conditions associated with aging.
- Care Consultation and Respite services is a main focus for this role and requires that ideal candidate to be able to discern between consultation, case management and coaching.
COMPENSATION & HOURS: This position is a full-time role, with flexibility in hours & schedules on a weekly basis. Compensation is based on hourly rate with bi-weekly timesheet, with mileage & expense reimbursement or stipend. Very flexible schedule and location; work at home or remotely. This position is based in Eagle County.
