TITLE: Behavioral Health Provider

LOCATION: Avon/Gypsum

REPORTS TO:   Behavioral Health Director

FLSA: Exempt 

Salary: $65,000 to $75,000 annually 

Sign on Bonus: $5,000

Geographic Pay: $5,000 annual bonus paid out annually in 26 pay periods.  

GENERAL PURPOSE:  Provides clinical assessment/problem identification, crisis intervention, individual, family and group therapy and case management. Documents interactions on medical charting software; consults with medical staff, billing staff and care coordination staff. Works with a diverse client population in a variety of treatment approaches and settings.                                           

ESSENTIAL DUTIES/RESPONSIBILITIES (Listed in descending order of importance)

Identify patents who could benefit from Behavioral Health services and coordinate with medical team to provide those Services          

Connect with referred clients during PCP visits (warm hand offs) skillfully and effectively.  

Provide Individual, family or group treatment using a solution Focused Brief Treatment Model.

Provide psycho-education in all areas of behavioral health and chronic disease management.

Accurately chart and enter data in the medical services system (electronic health record).

Effectively and efficiently consult with all care providers at Mountain Family Health Clinics. This needs to include competency in working with primary medical teams and a basic understanding of disease management principles; team based medical care, and coordination of care principles.

Provide excellent customer service to partner agencies, co-workers, community and clients.

Refer clients to appropriate community agencies.

Abide by confidentiality guidelines and HIPAA compliance standards.

Active involvement in clinic meetings and agency staffing to ensure continuity of care and ongoing program improvement.

Responsible for community outreach, networking, and resource development.

Conduct staff trainings as requested by Behavioral Health Director.

ADDITIONAL DUTIES/RESPONSIBILITIES

Comply with all MFHC Credentialing requests in a timely manner, including allowing access to NPI, and CAQH websites.

Comply with all Human Resource processes and programs

Complete all Human Resource assigned trainings

Miscellaneous Duties as Assigned

Perform such other duties or projects as determined by this positions supervisor.

JOB QUALIFICATIONS

Education or Formal Training & Experience: 

Master's degree in Counseling, Social Work, or Marriage and Family Therapy.  Will also consider a PhD, or PsyD in the mental health field. 

Licensed in the State of Colorado is required. (Will consider another state licensed if it can qualify for a Colorado state license.)

Applicants will be considered if they are soon to be licensed. 

Fluency in Spanish is highly recommended.   

Experience in the medical field will be a priority in the hiring process.

Knowledge, Skill & Ability: (include materials and equipment directly used)

Ability to produce written documentation of clinical work in a manner readily understood by other service providers.

Ability to demonstrate appropriate clinical judgment when assessing the level of risk and determining resolution of client emergencies, resulting in a safe outcome.

Experience providing behavioral services in a community based setting serving at risk and or underserved populations.

Experience with providing behavioral health services to a wide spectrum of ages and life stages.

Working knowledge of Windows based computer applications to include Microsoft Word, Excel, Outlook and internet access.

Ability to operate standard office equipment including computer keyboard, calculator, copy machine, fax and multi-line telephone.

Interpersonal and communication skills to develop and maintain effective working relationships with all internal and external customers.

Ability to organize and prioritize workload in a sometimes hectic environment with frequent interruptions.

Sensitivity to low income, ethnic minority community.

This position may require some travel between clinics.

Physical Requirements and Workplace Environment: 

Employee is frequently required to walk, sit, stand, kneel and have the ability to sit for longer periods of time at a computer. 

Must pass a criminal background check.

Annual Flu and Covid vaccine required. Employer will provide,

Obtain PPD for tuberculosis (annually if provider).  Employer will provide.

Obtain vaccine records for MMR, Varicella, T-DaP and Hep B, COVID-19 .   Employer will provide.
